WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?
We will provide you with a bill that you can submit to your insurance company for reimbursement of your out of network benefits. However each plan is different and the patient will be responsible for looking up their benefits. Each patient is responsible for paying at the time of their visit. We accept cash, credit, HSA cards, FSA cards or checks.
DO I NEED A DOCTOR’S REFERRAL?
No, you do not need a doctor's referral for physical therapy. Practicing as an out-of-network clinic in a direct access state means you can access physical therapy immediately without waiting to see the doctor.
WHAT IS YOUR NEW PATIENT PROCEDURE?
Before your first physical therapy appointment, we will send you an initial intake form for you to complete and discuss considerations for physical therapy treatment at your home or office, including pricing and pertinent paperwork.
On the day of your appointment, your physical therapist will arrive at your home or office and deliver a thorough physical therapy evaluation and examination followed by an assessment, diagnosis and treatment. An individualized plan of care will be developed that is personalized to your needs.
HOW MUCH DOES IT COST?
Pricing is based on the amount of time that your physical therapist spends with you with treatment times ranging from 15 minutes to 1 hour. Treatment time needed will vary based on the chronicity of the condition, number of body parts being treated, and individual needs of each patient.
